South Notts Moving Forward provides housing with support to people aged 16 and over with mental health needs.  It provides a range of support and is based at Arnold and Skegby and Sutton in Ashfield.
Description of service and support

South Notts Moving Forward consists of 4 houses, each having individual bedrooms and shared kitchen, living room and bathroom facilities. There are also shared gardens. In each living room there is a television for service users. There are 2/3 members of staff who staff the project from Monday to Friday, 9.00am until 5.00pm offering individual structured support,on call service available 24/7 when staff not on duty.  Cats and small pets are allowed.

Staff offer support to each individual, where support plans will be completed with the service users input. The support plans will be looking at small achievable goals to enable the service user to gain daily living skills to move on to live independently in the community. Support is also offered around education/ employment, social activities and sign posting to other agencies that service users may need to use.

Generally service users move on independent living with a referral for floating support if/as required.

Referral: To refer to Moving forward services, a CPN, GP or psychiatrist must put the referral to the Gateway worker at Framework, who will then forward on to Moving Forward services.
