Last Orders Intensive Case Management Service - Framework HA

The Last Orders Intensive case management service aims to engage the most problematic drinkers in treatment and to co-ordinate their care. The free service makes clinical and non clinical interventions to affect long-term behavioural change and it helps individuals to improve their lifestyles by resolving issues that result in problematic behaviour.

Description of service and support
Any patient referred to the service will undertake a comprehensive assessment and, if accepted, will be allocated a dedicated case manager who will work with all agencies involved to instigate long-term behavioural change. The Clinical Nurse Specialist co-ordinates all care and multi-disciplinary meetings and is supported by a band 5 nurse. When some stability is achieved the client is passed to support staff who work to instill sustainability of change.

Dependent drinkers classed as high volume service users: those who repetitively call ambulances, present at ED, occupy acute beds due to alcohol misuse. Patients will often have complex physical and mental health issues.
